Consider introducing a middle-layer:

passive: e.g. for adapting DICOM messages by using a scripting language. Whenever a message passes by, it is adapted according to the rules.
active: a user wants to keep two archives in sync, the active layer could e.g. observe regularly, or use the FHIR subscriptions to get notified of changes and perform the change on the other archive, too.
